docs(changelog): add 0.95.0 entry

0.95.0 was published (tag fe37277) but CHANGELOG.md stops at 0.94.0. Adds the missing entry, derived from the v0.94.0..v0.95.0 commit range:

• Added — FAPO proposer policy (gepaProposer, src/campaign/proposers/gepa.ts) for proposer-driven runImprovementLoop / runCampaign.
• Changed — profile handling finalized on the canonical @tangle-network/agent-interface AgentProfile; per-run cells via buildAgentInterfaceProfileCell (kind agent-interface-profile).
• Removed — pruned stale public exports; loop/proposer/gate primitives now resolve via the @tangle-network/agent-eval/contract subpath, not the package root.

Doc-only; pnpm typecheck clean (pre-commit hook).
